Informed Consent Form
You are invited to participate in a study conducted by G.L.A Harris and Rose M. Jackson of Portland State University. The researchers hope to learn about your experiences as officers in the United States Navy. You were selected as a possible participant in this study given your assignment as an officer who has served at one time or another during your Naval career on sea duty.
If you decide to participate in this study, you will be asked to complete the attached questionnaire as the first part of the process. Following your completion and based upon your response to the questionnaire, you may be contacted to participate in a focus group to discuss the findings of the study.
Upon completion of the questionnaire:
- No attempt will be made to link your name to any particular responses.
- Should you agree to also participate in any of the focus groups, any information that is obtained in connection with this study that can be linked to you or identify you will be kept in the strictest of confidence.
- This information will be kept confidential through storage in a safe and secure place where only the researchers of this study will have access.
- Neither your name nor your organization (i.e., ship assignment) will appear in any of the reporting for this study.
Your participation is voluntary. You do not have to take part in this study. Declining to participate will in no way affect your relationship with the researchers, Portland State University or the United States Navy. Further, this study is independent of and is not sponsored by the United States Navy and/or its affiliates. The purpose of this questionnaire is to collect information concerning organizational change and behavior within the United States Navy. You may withdraw from the study at any time without affecting your relationship with the researchers, Portland State University or the United States Navy.
Public Law 93-579, called the Privacy Act of 1974, requires that you be informed of the purpose of this survey and of the uses to be made of the information collected. Authority to request this information is granted under 10 U.S.C. 5031 and 5032, and 5 U.S.C. 301: Executive Order 9397. License to administer this survey is granted per OPNAVINST 5300.8B under OPNAV Report Control Symbol (RCS) 1301-1a, which expires August 31, 2008.
PURPOSE/ROUTINE USES: The purpose of this survey/questionnaire is to collect information concerning Navy female officers' experiences on ships. The researchers are examining organizational change and behavior within the Navy.
