The Oregon Master of Public Health Program (OMPH) invites you to participate in our Alumni Survey.  The OMPH Program continues to grow and evolve to meet the needs of students and the public health workforce.  One of the ways that we continue to improve is by incorporating your valuable comments into our strategic plan.  This information will  be used to improve career guidance and services for students and for ongoing accreditation reporting.

We would very much appreciate your taking a few minutes to complete this survey. Your participation is voluntary, and your responses through this electronic survey instrument are anonymous.  Any identifiable information you provide will be kept strictly confidential, and your email address will not be connected with your survey responses.  This survey should take approximately 5 minutes to complete.

As an incentive, four survey respondents will be selected at random to win a $40 gift card to Powell’s Books, to us in-store or online.  The winner will be announced by email upon closure of the survey.
